This form affirms that an applicant has completed the Application for a Disability Exemption and submitted it to the Office of the Jefferson County Property Valuation.

This form also verifies that the information regarding the applicant’s total disability is factual and true.

The form also verifies that the applicant has been the owner of the stated property and that the applicant has personally, and continually resided in said residence during the tax year for which the exemption is being applied.

The Affidavit for Total Disability Verification further states that the applicant has no other residence and that the applicant has maintained the same classification of total disability since December 31, of the preceding tax year.
